To begin the process for working 1 on 1 with me, please follow the steps below. This screening process is important and unfortunately, not all clients will be able to work with me and this depends on multiple factors. 

All communication prior to being a client will be via email. Please be sure to check your inbox for emails from me. If you do not see any email communication, please remember to check your junk folder. If you still have not seen anything, please feel free to send an email to [email protected] to inquire about your pre-screening forms. 

I do a lot of communication via email and texting. Therefore, please do take this into consideration if you want to work with each other and before you complete the pre-screening forms. 

If you are looking for just Chinese bodywork (tui na/massage), please complete the appropriate form labeled below.

To see if you qualify for a case review:
Step 1 pre-screening:
  • Click on the appropriate female/male pre-screening form below and complete the form. There is only 1 submission therefore, please allow enough time to complete the form in full.
  • If for any reason, you do not receive a follow up message upon submitting your pre-screening form, please reach out to me via email. 

Step 2 pre-screening:
After I have reviewed your forms and have made a decision to move forward, there will be a second pre-screening. An email will be sent to you regarding the screening. 

Case Review:
Once your pre-screening process is completed and I have decided to move forward, you will be contacted to set up your case review. The case review is the session where I focus on gathering your health history and review your medical records so that I can determine what the next crucial steps should be in your total health transformation. Once booked for your case review, it is important that you show up. Failure to do so will result in immediate termination from the clinic and you will not be able to book for another appointment. A debit or credit card is required upon booking for your case review. A fee of $200 will also be applied for no show or less than 24 hour cancellation of a case review appointment.
